scam-backscatter milter

Scam-backscatter prevents spam backscatter (accept and bounce) on
mail servers which don't host mailboxes locally. It validates
mailboxes by verifying the recipient addresses hosted on a different
mail server.

mail category: Remove $PTHREAD_LIBS

Note: The following ports did not pass check-plist (CP) or stage-qa (QA)
tests. The sendmail port has many problems, but this was already known
by myself as I fixed it properly on DPorts last week. It's a forward
action to submit a PR against mail/sendmail.

  * antivirus-milter (CP)
  * dk-milter        (CP)
  * milter-regex     (CP)
  * sendmail         (CP, QA)

In the rc.d scripts, change assignments to rcvar to use the
literal name_enable wherever possible, and ${name}_enable
when it's not, to prepare for the demise of set_rcvar().

In cases where I had to hand-edit unusual instances also
modify formatting slightly to be more uniform (and in
some cases, correct). This includes adding some $FreeBSD$
tags, and most importantly moving rcvar= to right after
name= so it's clear that one is derived from the other.
