Update for port net/asterisk-app-ldap

        Update for port net/asterisk-app-ldap. The new version runs
        now with asterisk 1.4.

Also modified PORTNAME to asterisk-app-ldap and introduced DISTNAME

Switch to using net/asterisk12 and net/asterisk12-addons in preparation
for asterisk upgrade to 1.4.x.

Unbreak building. I had the needed patch while testing at my
tinderbox but committed the wrong port version.
