Update for port net/asterisk-app-ldap

        Update for port net/asterisk-app-ldap. The new version runs
        now with asterisk 1.4.

        please rename the old port asterisk-app-ldap to asterisk12-app-ldap
        and commit this port as asterisk-app-ldap

Also changed PORTNAME to asterisk12-app-ldap and introduce DISTNAME.
