Add missing IGNORE_WITH_PHP for various PHP based ports.
In preparation of changing the PHP default version from 5.6
to 7.0 i found a number of ports not building with the new version
and all later versions.
Therefore i set IGNORE_WITH_PHP and notice all maintainer about
the change.

According to the Porter's Handbook (5.12.2.3.) default options must be added to
OPTIONS_DEFINE. This policy has been implemented only recently that's why we
have many ports violating this policy.
This patch adds the default options specified in the Porter's Handbook to
OPTIONS_DEFINE where they are being used. Ports maintained by
gnome@FreeBSD.org, kde@FreeBSD.org and x11@FreeBSD.org have been excluded.

Remove more tags from pkg-descr files fo the form:
- Name
em@i.l
or variations thereof. While I'm here also fix some whitespace and other
formatting errors, including moving WWW: to the last line in the file.
- Update to 2.2 RC2a
The patch in PR updates to 2.2 RC2, which is not available now. So I
directly update it to RC2a.

- Fix dependencies (php, apache, mysql, what a mess, it depends only on php)
- Fix pkg-plist
- make portlint happy
- add SIZE
- remove BROKEN and DEPRECATED
Approved by: nork (mentor/implicitly)
BROKEN: inconsistent dependencies on mysql 3.x and 4.x (inherited from a
dependency). This port will be removed on April 15 if still broken at
that time.
