Remove # $FreeBSD$ from Makefiles.

In 1998, Markus Oberhumer greatly extended this program and bumped
the version to 0.4.  Finally switch to a tarball that incorporates
those changes and stop carrying them along as a patch set.

Add USES=xorg USES=gl, ports categories x

Add USES=xorg and USES=gl to ports in categories starting with 'x'
While here, try to sprinkle other USES (mostly gnome and sdl) as needed.

MASTER_SITES cleanup.

- Replace ${MASTER_SITE_FOO} with FOO.
- Merge MASTER_SITE_SUBDIR into MASTER_SITES when possible. (This means 99.9%
  of the time.)
- Remove occurrences of MASTER_SITE_LOCAL when no subdirectory was present and
  no hint of what it should be was present.
- Fix some logic.
- And generally, make things more simple and easy to understand.

While there, add magic values to the FESTIVAL, GENTOO, GIMP, GNUPG, QT and
SAMBA macros.

Also, replace some EXTRACT_SUFX occurences with USES=tar:*.

Rename x11/ patch-xy patches to reflect the files they modify.

- Convert to staging

Add NO_STAGE all over the place in preparation for the staging support (cat:
x11)

Convert to USES=imake
Trim headers
Convert emu to USES=display
Remove indefinite article

- Get Rid MD5 support

Convert most of remaining ports that depend on xorg-libraries to
modular xorg.

- supply corresponding USE_XORG for all imake-using ports that need it
- USE_IMAKE no longer implies USE_XLIB in absence of USE_XORG
- retire USE_X_PREFIX which is not really used anywhere after the
  above change
- a few minor nits like whitespace and SF macro

new virtual category requested: accessibility

        This category, to be described as "of interest to disabled
        users" is intended to make it simpler for users to find
        software which will let them overcome a handicap.  For
        example, blind users may want to combine OCR and text-to-speech
        software so the computer can read books to them; users with
        diminished eyesight can use large fonts and "magnifiers";
        paralyzed users may use speech recognition to compose text
        or give commands.
